A 56-year-old male presented to the ER with septic shock, fever, and right flank pain. Emergency physicians (EP) used bedside ultrasound to diagnose pyonephrosis (PN) caused by a distal ureteral stone. Immediate antibiotic treatment and surgical intervention with a ureteral stent were initiated, leading to rapid recovery. The case highlights the critical role of emergency ultrasound in the early diagnosis and management of life-threatening conditions like PN in the emergency department.
