This study aimed to evaluate the accuracy and benefits of an ultrasound-guided technique for iliopsoas peritendon or bursa injections as an alternative to the conventional fluoroscopic guidance. The research included patients with a history of total hip arthroplasty (THA) who were referred for corticosteroid injections due to iliopsoas impingement syndrome. Between June 2017 and December 2019, 19 patients received 26 ultrasound-guided needle placements followed by fluoroscopic confirmation before injection. Pre- and post-injection pain levels were monitored using the Visual Analog Scale (VAS), and patients were followed for six months to assess complications, the need for repeat injections, or progression to surgery. The study utilized a longitudinal in-plane distal-to-proximal approach for ultrasound-guided needle placement, which was evaluated for accuracy by fluoroscopic confirmation of contrast material in the intended anatomical site. The results showed that ultrasound-guided needle placement using this method effectively delivered the injection to the correct location, making it a promising alternative to fluoroscopy. The technique was not only accurate but also advantageous in reducing radiation exposure for patients. The study concludes that this ultrasound-guided approach is a reliable and efficient technique for diagnostic and therapeutic iliopsoas injections in patients who have undergone THA, offering a viable alternative to traditional imaging methods.
